Care for an 82-year-old lady, 50 kg, 165 cm, living independently, after a stroke, who is mentally well and able to move independently despite her health limitations, but at risk of falling. Her condition requires caution, especially when standing up, as she appears dizzy when moving quickly. Medically, it is a combination of several diagnoses: post-stroke condition with overcoming delirium, polyneuropathy, macular degeneration (impaired vision), hearing impairment (hearing aids – need to speak clearly and patiently), COPD with the need for inhalation approximately three times a day and heart failure (stabilised by medication). The lady is partially self-sufficient but needs supervision and assistance with activities of daily living. This includes assistance with personal hygiene, dressing and accompanying her to the toilet. She eats independently, but needs to be motivated and supervised to ensure sufficient fluid intake. Support with inhalation, supervision of medication and general health monitoring are important parts of care. Walks may also be part of the day, but only with an escort and according to current condition. The lady regularly attends physiotherapy and occupational therapy, to which she needs to be accompanied. The caregiver also takes care of the running of the household such as cooking, shopping (within walking distance), cleaning and laundry. The lady can be involved according to her condition. Aids such as a wheelchair (especially for moving around outdoors) and a shower chair are provided. Night work is not required by default, the lady goes to the toilet on her own, but it may be appropriate to have occasional supervision according to current condition.
