Care of an independent living 93 year old gentleman with old age weakness , 53 kg, 168 cm, following a recent gastrointestinal infection and hospital admission in February 2026, who is currently physically frail, has difficulty walking and getting up and also shows mild signs of dementia. The gentleman moves very slowly with a walker, getting up is challenging and dependent on sitting height. There is an increased risk of falling, therefore supervision and assistance with movement is required. Hearing is severely impaired, so there is a need to speak louder and more clearly. The gentleman suffers from urinary incontinence and uses incontinence aids, changing them is the responsibility of the Caregiver. He is allergic to certain foods (dairy products: cheese, cottage cheese, yoghurt). She requires assistance with basic daily tasks such as getting up, lying down, transfers, personal hygiene, dressing and supervising fluid intake. He needs support with transfers and mobility, but is not completely immobile. Part of his care includes working with an outpatient nursing service to ensure medication is administered and compression stockings are put on. The job also includes housekeeping, cleaning, cooking ensuring shopping(within walking distance) and normal daily activities such as supervision, activation, communication, maintaining a daily routine and companionship. Night care is only occasional, the gentleman can call for help if needed. There is a hoist, walker, bath chair.
