Care for an 86-year-old gentleman, 176 cm, 75 kg, suffering from dementia, Parkinson's disease, and heart condition. Despite dementia, he is fully oriented. The gentleman is mobile and moves independently, with a walker available if needed. There is a risk of falling. He moves independently when walking. Assistance is needed when walking on stairs, bathing, showering, dressing, and undressing. The gentleman suffers from partial urinary incontinence, especially at night. He uses incontinence aids, which the caregiver changes. Assistance with toileting is needed as required. Daily assistance is needed for personal hygiene, including body washing, intimate hygiene, dressing, and undressing. The caregiver should remind him to take his medication, monitor fluid intake, and assist with toileting as needed. An outpatient care service visits in the morning and provides morning hygiene. This primarily involves assistance with toileting. The duties also include cooking, shopping, and general household help. The gentleman lives with his wife, who only requires occasional household assistance. Family members live nearby but will not visit the senior regularly during the agreed-upon replacement period. Shops are approximately two kilometers away. Public transport (bus and train) is available. The household has an adjustable bed, a commode, and a barrier-free adapted bathroom. Nighttime care is needed irregularly, sometimes multiple times a night depending on the situation. Lifting is not required. Free time is by arrangement. The caregiver has a private bathroom available.
