Care for an 84-year-old gentleman, 59 kg, 178 cm, suffering from Parkinson's disease, dementia, urinary incontinence, and high blood pressure. He recognizes people and is oriented in his home environment; he is also oriented outside the home, but his temporal orientation is limited and his short-term memory is gradually deteriorating. He occasionally has hallucinations, most often seeing animals or other harmless things that do not actually exist, but he can be easily reassured. Communication is maintained; he can express his needs and engage in simple conversation. He moves around the household with the help of a walker; for longer distances outside the home, he uses a wheelchair. He needs assistance when walking up stairs and there is an elevator available to the apartment. On some days, he needs help getting out of bed or a chair. The gentleman requires partial assistance with dressing, personal hygiene (showering, washing hair, shaving, and getting into the shower; a shower chair is available), and toileting. He tries to cooperate and performs activities independently as much as possible. He manages his meals himself; it is necessary to ensure adequate fluid intake and regular shared meals. The duties include meal preparation, grocery shopping, cleaning, household care, accompanying him to doctor's appointments as agreed with the family, supervising medication intake (which is pre-prepared), assistance with daily organization, companionship, and communication. He lives at home with his wife and 62-year-old son. No care is needed for the son or wife. The son takes care of his own laundry and cleaning and goes to work. Night work or heavy lifting is not required. Free time is arranged by agreement. A car is available.
