The 86-year-old lady, 80 kg, 163 cm, has early-stage dementia, asthma, allergies, high blood pressure, heart failure, arthritis, and scoliosis. She suffers from sleep disorders and anxiety. Both have partially impaired orientation, vision, and speech, and use glasses. Both have partially limited mobility for walking and standing. They use a cane, a walker, and an adjustable bed. The gentleman also has a toilet seat riser. They cooperate during transfers; lifting is not required. They need assistance with showering, skincare, dressing, and partially with toileting. They use incontinence pads; incontinence is not specified. The gentleman also needs help with shaving and nail care. Both eat independently. The lady has a poor appetite, difficulty chewing, and low fluid intake, thus requiring supervision of her fluid intake. Medications are prepared by the family. A nursing service visits twice daily for compression stockings. Night shifts are not required. Both need support in organizing their day, activation, and companionship.
